Wakes at 3am, dry mouth, palpitations on waking, mild night sweats, intense dreams. Tongue: red tip with center crack, scanty yellow coat at root. Pulse: thready and rapid on the left chi, slippery on right guan.
Diagnosis
Kidney Yin Deficiency with Empty Heat harassing the Heart, complicated by Latent Heat in the Shao Yin layer.
TCM Reasoning
Classical 'Heart-Spleen' presentation misleads, the 3am wake (Liver hour, but with dry mouth and chi pulse weakness) plus the cracked center and rooted yellow coat point to Yin depletion at depth. The Empty Heat ascends to disturb the Shen at night. Prior practitioners treated the branch (Heart) not the root (Kidney).
Western Correlation
Likely peri-menopausal HPA axis dysregulation with low nocturnal melatonin and elevated 3am cortisol. Recommended: 4-point salivary cortisol, FSH/LH, estradiol, and a sleep study to rule out REM-stage fragmentation.
Treatment
KI3 + KI6 (nourish Yin, anchor Shen) · HT6 (Yin Xi, night sweats) · SP6 + LV3 to cool Empty Heat · Yin Tang + An Mian. Herbs: Tian Wang Bu Xin Dan modified with Huang Bai + Zhi Mu (Zhi Bai Di Huang influence) for the Shao Yin Heat. Avoid Suan Zao Ren alone, too sour for this Yin depletion.
What to tell the patient

Case 2: The Atypical Bell's Palsy
34yo Male, 8 months post Bell's Palsy with 60% recovery plateau. Persistent synkinesis, eye watering on chewing, and a 'pulling sensation' on the affected side worse in cold wind. Tongue: pale with thin white coat, slight bluish tinge on the sides. Pulse: tight and slightly wiry on the left, deficient on the right cun.
Diagnosis
Residual Wind-Cold in the Yang Ming and Shao Yang channels with secondary Blood Stasis in the facial collaterals (Luo), on a background of Lung Qi and Wei Qi Deficiency.
TCM Reasoning
The plateau and synkinesis aren't 'just nerve damage', they're a sign that exogenous Wind has lodged in the collaterals and that local Blood Stasis is now obstructing aberrant nerve regeneration. The bluish tongue sides and tight pulse confirm stasis. The right cun deficiency explains why the Wei Qi never fully expelled the pathogen, the Lung defense was already weak.
Western Correlation
Aberrant regeneration of CN VII (synkinesis) with possible underlying immunodeficiency (recurrent URTIs). Recommended: EMG/nerve conduction to map fiber regeneration; consider IgG subclass panel if recurrent infections.
Treatment
Local: ST2, ST4, ST6, SI18, GB14 with shallow threading + plum-blossom tapping. Distal: LI4 + ST44 (Yang Ming), TE5 + GB41 (Shao Yang). Add LU7 + LU9 to tonify Wei Qi. Bloodletting on visible vessels at Tai Yang. Cupping over upper back (BL12-BL13). Avoid electroacupuncture on the affected side, worsens synkinesis.
What to tell the patient
"Think of your facial nerves like tangled headphone cables in a drawer that hasn't been opened in months. Just pulling harder won't fix it, we need to gently open the drawer, untangle each wire, and clear out the dust that got in. That's what we're doing: releasing what's stuck, then teaching the wires to fire in the right order again."
